I am not sure the point of your question.
I agree that the following is a valid ecoding:
Mary yelled at her sisters, <q>Mom said, <q>Go to sleep now!</q>  And if
you don't, I'm gonna tell!</q>

I would prefer:
Mary yelled at her sisters, <q level="1">Mom said, <q level="2">Go to
sleep now!</q>  And if you don't, I'm gonna tell!</q>How it is rendered 

We could also encode:
Mary yelled at her sisters, <q level="1" who="Mary">Mom said, <q
level="2" who="mom">Go to sleep now!</q>  And if you don't, I'm gonna
tell!</q>

It could be rendered in any typographic style to indicate the encoded
information.  The most common being the following:
Mary yelled at her sisters, "Mom said, 'Go to sleep now!' And if you
don't, I'm gonna tell!"
