Who lives in the state of California?

I've forgotten the statute, but it is an offense to sell goods or
services in the state of California, without disclosing one's physical
address, and complete contact information.  IIRC, their AG's working
assumption is that failure to provide that information is prime facia
evidence of intent to defraud consumers.

Directly,no.  Indirectly, yes.

If the organization does not provide source code, then a DMCA take
down notice can be filed with the webhost, because they are
distributing the program in violation of the license.

Whether or not Crosswire Bible Society wants to get involved in a
lengthy criminal trial or not, is a slightly different question.
