Agree, that is why I suggested the milestone solution.

 From the standpoint of XML syntax (which may explain in part our 
divergence on this issue) <p/> = <milestone type="p"/> or rather 
<milestone type="x-p"/>. There is no difference in those two, well, the 
GI (generic identifier) that is the "p" or "milestone" differ but the 
semantics are the same.

In other words, I think <milestone type="x-p"/> captures the intent of 
Lockman, which is to indicate a paragraph break at that location in the 
text.

Why is this contrary to allowing them to encode their Bible? I have not 
seen the underlying code but assume that from the manner in which you 
describe your encoding, they have chosen another hierarchy other than 
book/chapter/paragraph. That is a legitimate choice but is at variance 
with our decision to make other hierarchies give way to that one.

Granted that means that what Lockman may "think" is the most natural way 
to convert their text does not work but so would book/chapter/verse for 
publishers who go the other way. At some point, unfortunately with the 
present state of XML, one has to pick a hierarchy and stick with it. All 
others must give way.

If you think you are disappointed by that, imagine how I feel after a 
decade of working on overlapping hierarchies and there are still no 
standard implementations for what is probably the most common cases, 
like paragraphs that cross page boundaries. Not to mention more 
complicated cases.
