Index: identify important words or occurrences of words so
an index can be created automatically.

Subject index: identify a word, phrase, or location in the document 
as a point that should be added to the subject index under a 
certain head/subhead.

Glossary: it's like a list except that the lists aren't identified
as terms and definitions. So processing software wouldn't know to
use that list as a way of looking up terms.

Hmmm, interesting question. I assume to construct indexes from terms so 
marked to avoid auto-generation? In other words, the encoder marks what 
they feel are the important occcurrences of a term and ignore the rest?
