thml2gbs has some bugs and is broken at the moment, I believe.

You should not modify the .bdt files manually because it will mess up the 
indexing in the other files.  There are some quirky things you COULD do in 
ThML (or OSIS or any other XML format) that would really confuse thml2gbs, 
but I don't think that is done in any texts I've seen.  So any problems 
you're having are probably the result of either:

1) bugs in thml2gbs (it was really just a proof-of-concept project and 
didn't quite get finished)
2) bugs/inadequacies in Sword's or BibleCS's handling of ThML
3) possibly bugs in the GenBook code, but this is least likely
