[osis-core] namespaces

In simple terms all elements must be declared in the OSIS namespace
(http://www.bibletechnologies.net/2003/OSIS/namespace).  

You can either declare the OSIS namespace to be the default as in 
<osis xmlns="http://www.bibletechnologies.net/2003/OSIS/namespace" ...>
OR use a prefix for the namespace as in 
<osis:osis
xmlns:osis="http://www.bibletechnologies.net/2003/OSIS/namespace" ...>

Both forms are correct, I favor the simpler default that Troy posted.  

The user manual has not been updated to reflect the post OSIS 1.1.1
schema.
