[osis-core] Element Review: <figure>

The content model doesn't seem to allow a <figure> in a <p> -- should 
it? How about in a <note>?

<figure> has: no content, just reference to image source and <caption> 
(optional).

Should this be amended to allow <note>? Seems like having notes on an 
image would be fairly common.
