We use JIRA at work, and it's fairly decent. There are a few things that 
irritate me about it, but then I'm irritated by almost all issue trackers out 
there.

Another good one (which you can even create tickets via e-mail) is Redmine. 
The OpenLP project is using redmine for their support@ and bugs@ e-mail 
addresses.

(Their actual bug tracker is at Launchpad, but the Redmine one provides a way 
to reduce the number of duplicate bug reports.)
