I have seen this bug in the past. It seemed intermittent. Currently I 
have not seen it for some time and I don''t know how to reproduce it. I 
have looked at the code that initializes the main menu and toolbar and I 
don't see anything that looks wrong. Does anyone know how to reproduce it?
