I believe it's called kspell, and it will use either aspell, ispell, or a 
couple of others and which one it uses is set in the control panel. I know 
people are working on kspell2 but I don't know what kind of support that 
provides atm. But you can have a fully integrated spell checker without much 
work via kde even with on the fly checking.

Currently there are none, there was a test module that had them just to show 
off the functionality. The problem is that the install manager for sword (the 
backend library) doesn't support downloading the image directory for the 
modules (yet) But it'd be like in the ISBE you go to the article on Aaron and 
get his picture would be a classic usage. But more importantly, you could 
have a lexicon of maps and each entry be a static map of place X.
